Metabolic Syndrome

Not a disease of excess calories but of corrupted substrate signaling. The central insulin resistance is primarily molecular, not mechanical.

The Substrate-First Model

Conventional model: Overconsumption → adiposity → inflammation → insulin resistance

Substrate model: Dietary oxidized lipids → IRS-1 adduct formation → insulin resistance → compensatory hyperinsulinemia → adipogenesis → further inflammation

The distinction matters because it predicts:

  1. Metabolic syndrome without significant obesity (lean NAFLD, lean PCOS)
  2. Persistence of metabolic dysfunction after weight loss (the adipose reservoir continues releasing OXLAMs)
  3. Response to substrate change independently of caloric change

The Adipose OXLAM Reservoir

OXLAMs accumulate in adipose tissue and continue releasing:

  • TNF-α, IL-6 (maintaining systemic inflammatory baseline)
  • OXLAMs themselves (re-entering circulation from stored reserves)
  • Sustaining macrophage M1 polarization in adipose tissue (crown-like structures around dead adipocytes)

This is why metabolic improvement lags dietary change — the reservoir is not instantly cleared even when dietary intake drops to near zero.
