Metabolic Syndrome
Not a disease of excess calories but of corrupted substrate signaling. The central insulin resistance is primarily molecular, not mechanical.
The Substrate-First Model
Conventional model: Overconsumption → adiposity → inflammation → insulin resistance
Substrate model: Dietary oxidized lipids → IRS-1 adduct formation → insulin resistance → compensatory hyperinsulinemia → adipogenesis → further inflammation
The distinction matters because it predicts:
- Metabolic syndrome without significant obesity (lean NAFLD, lean PCOS)
- Persistence of metabolic dysfunction after weight loss (the adipose reservoir continues releasing OXLAMs)
- Response to substrate change independently of caloric change
The Adipose OXLAM Reservoir
OXLAMs accumulate in adipose tissue and continue releasing:
- TNF-α, IL-6 (maintaining systemic inflammatory baseline)
- OXLAMs themselves (re-entering circulation from stored reserves)
- Sustaining macrophage M1 polarization in adipose tissue (crown-like structures around dead adipocytes)
This is why metabolic improvement lags dietary change — the reservoir is not instantly cleared even when dietary intake drops to near zero.
